Changing your Care Plan

If your care needs change with time then it is important that together we review and revise your care plan. Please tell your carer or the SureCare office (01924 404570) if you think that is needed.

If you think you need an increase in the number of hours of care you receive together with  extra financial support towards the increased cost then you will need to have your care plan  reassessed by a social worker. 

To request that reassessment you will need to contact the Social Services Information Point (SIPS). Their telephone numbers are at the end of the Service User  Guide we have given you and which can also be seen within the "Care Services" pages of this website.

If you need more care and intend to pay for the extra cost yourself without seeking financial support from elswhere then you do not need to be reassessed by a social worker. We will carry out that reassessment with you.

Please contact us at the earliest possible opportunity if you are considering changes to your care plan. We will advice you of the further actions that may be necessary.
