Care Plans

Service user care is based upon care plans, initially drawn up at assessment after referral. The preparation and monitoring of care plans directly relates to the attainment and retention of normal daily living activities. They also highlight any risks or hazards to yourself or the carer.

Care plans will be reviewed on a regular basis with the service user, their relatives and/or their representatives. The objectives and methods of care plan achievement will be agreed with the service user.

Within this care plan we aim to ensure that service user rights are observed. These may be summarised as the right to choose, to privacy, to self-expression, to be an individual, to independence and to respect.

The way that care is provided and organised should enable the service users to feel valued as an individual, preserving their independence and helping to retain their place in the community.
